Interprof HOME - Development and testing of an interprofessional person-centered care concept for patients living in their own homes
In view of the often complex care of people in need of care in outpatient home care, coordination between care services, family doctors, therapists and family caregivers is often inadequate. This can have a negative impact on patient safety. Currently, approximately 50% of hospital admissions in this patient group are due to "ambulatory-sensitive" diagnoses.
Aim of the project:
The aim of this study is to develop a demand-oriented, interprofessional patient-centered care concept to avoid hospital admissions. In addition, it aims to reduce the use of unplanned medical or rescue services and to strengthen the improvement of the quality of life of patients* in need of care at home.
