Summary:
As of today, Cardiovascular diseases (CVD) are still the leading cause of death in Germany. Hereby, the separation in Germany between inpatient and outpatient care is one of the major obstacles to the availability of patients’ health information. CAEHR aims at establishing a hub that will enhance health care through cross-sectoral routine data integration. The goal is to improve cross-institutional and cross-sectoral communication between different health care actors and leverage longitudinal health information to automatically assess risk factors and early signs for critical developments in the patient's individual disease.
Within this context, the DHRG will conduct an analysis of different solutions to determine how the hub can be operated in a long-term and sustainable manner.
